We know your time is valuable, so we have included our clinic paperwork on this page for your convenience. The forms are double-sided so you may set your computer to duplex printing if you have that option.

In an effort to provide the best possible care for our patients, it is required that we receive all new patient paperwork and laboratory results at least 2 business days prior to your initial visit. If you are scheduled for lab work only on your first visit, you may bring in the new patient forms at that time.

LABORATORY WORK

New patients need to have lab work prior to their first visit. For your convenience, we can draw your labs in the clinic. We keep our cost amazingly low by requiring out-of-pocket pay and not going through insurance companies. See our costs listed below.

If you have lab work that has been done within the past 3 months, you may fax or send it to our office for review prior to your first visit. We will let you know if additional lab work is needed. If you choose to do lab work through your primary care physician, these are the minimum labs we would like to see. Reminder: You need to be fasting for at least 12 hours prior to your blood being drawn, although be sure to drink adequate amounts of water.

Weight Loss Patients
$135 or $165 including Vitamin D

CBC - complete blood count
CMP - complete metabolic panel
Lipid panel - cholesterol, triglycerides, LDL, HDL
Thyroid panel - TSH, free T3, free T4
25(OH) Vitamin D - optional, but highly recommended


Hormone Balancing Patients

You will need all labs listed above, in addition to a variety of hormone levels. These vary based on whether you are male or female and if serum or saliva is being tested.
